Education Health Care Plans 

To support the completion of paperwork for Education, Health, and Care (EHC) Plan Reviews, a step-by-step guide is provided at the bottom of this page. This guide explains what documents will be sent out, when they will arrive, and which sections need to be completed and returned. 

 EHC plans focus on the aspirations of each child or young person, outlining the support required to help them achieve their goals. Each plan is personalised to meet your child’s specific needs, so every EHC plan is unique. It includes details about your child’s preferred communication style, the support they need, and the outcomes they wish to achieve. 

EHC plans are reviewed annually, offering a chance for all involved parties to make updates, adjust goals, and modify support as needed.

Year 9 onwards - Preparation for Adulthood 

Support, advice and guidance is provided throughout discussions at EHCP Reviews from Year 9 onwards with different pathways into further education and employment being explored. Please see the attachments below, which include useful information and both parent and pupil view documents.
